ManageHF4Life is a symptom tracking and educational app for patients with heart failure. It is designed to help patients monitor and manage the symptoms of heart failure. It is based on evidence that self-monitoring can improve patient outcomes and quality of life. The app was designed by researchers at the University of Michigan.

ManageHF4Life provides:
